Regarding Background Check

The above document is my most recent DOJ caregiver background check. I have to do this every two years for our parish process.

Regarding Ordination

Diaconate Ordination Oct 6, 2001
This was more of a regularization due to having been conditionally ordained a deacon by my Roman Catholic Diocese to aid an ailing priest in the diocese. No certificate was ever provided.

Priestly Ordination Oct 7, 2001

Letter announcing ordination

There was a question on the application about Apostolic Succession. I answered “No” as I am not a bishop; however, I found the information in the file below during a recent internet search. Please note that though my ordination certificate states I was ordained by Bishop Michael Norton, two other bishops were present for my ordination – Presiding Bishop Lawrence Harms and Bishop JJ Johnson. I do have photos of their presence at my ordination if needed. (which I found a little unorthodox at the time t have three bishops ordaining a priest. The presiding bishop later told me he had the intent to ordain me to a higher station but was unable to follow through as I was not yet a priest. I always assumed he was speaking in jest.)

The +Lawrence Harms Lineage of Consecration (which includes Bishop Norton).

Regarding Education

Please note: my seminary experience was not a good one. I attended Sacred Heart School of Theology, which is mentioned in the book, “Good-bye, Good Men” as has having been an institution that gave “favor for favor”. My grades, at the time, reflected that atmosphere.

This is a large comprehensive file with all available transcripts. I do not currently have scanned copies of my diplomas. I have never needed them so I framed them almost immediately. I will scan them if they are required, however, the transcripts show the conveyance of degrees.

Regarding Licensing as a Professional Counselor

In 2018, Wisconsin Law changed in that current LPC’s with substance abuse counseling experience and/or training may counsel as a SAC under their current license. Therefore, I will not renew my SAC license in the next cycle.

This is my current License Professional Counselor.

Same license, though dated

This is the National Board of Certified Counselors certification (NCC).

Regarding Identity

A copy of my identification cards. Please note – I am a dual citizen of the United States of America and of the United States of Mexico. My mother was a Mexican National – I was naturalized in Mexico as a foreign-born citizen due to having been born in the United States.

If you need anything else, please let me know and I will get those to you as quickly as possible.
